The West Virginia Motor Vehicle Power of Attorney (Form DMV-9-TR) can be used by motorists in West Virginia who want certain tasks handled by a representative of their choosing. The West Virginia DMV permits a vehicle owner to authorize an agent to attend to matters that are normally handled in person by the vehicle owner. 2. The identification details of both the owner and agent of the vehicle Should Be Filled Out

This document begins with an empty line showing the complete name of the owner of the vehicle. The following line “…Constitute And The following line “...Constitute And Appoint” identifies the agent who will represent the principal. 

3. Where requested, provide the vehicle's identification information

Under the statement of declaration, there are two checkboxes: “Motor Vehicle” and “Mobile Home.” Select the box that corresponds to the Vehicle. Item number two here needs materials provided in a tabular form. On the table's first row, there will be labels that identify vehicle information. Here you will document the "Vehicle's Model," "Make," and "Body Type," as well as the "Title Number."

The second row will display the Identification Number of the vehicle. To present this information clearly, separate boxes have been provided for each digit of the VIN.

4. This delegation of authority must be signed by the vehicle owner

The owner (s) of the principal vehicle must sign the "Signature Of Owner(s)" line after providing all the information above. A signature from each owner is required here. The Vehicle Owner's address, city, state, and zip code will appear below the signature(s).

As soon as the documents are completed and signed, they must be presented to the Notary Public at the signing. Several items will be provided to this document by the notary so that it can be properly notarized.
