Healthcare Advance Directive- It combines the functions of a living will and a medical power of attorney, allowing a patient to indicate their choices for end-of-life care as well as the person they wish to represent their interests in medical decisions on their behalf.

Requirements needed to sign the document- The signature must be witnessed by a minimum of two (2) individuals.

Statutory long-term (Durable) Power of Attorney- Create a durable (statutory) power of attorney that is confined to financial affairs if you want to protect your assets from creditors. Even if one side violates an agreement, the agreement remains in effect.

Requirements needed to sign the document- In accordance with Section 21-2101, notary public signatures are necessary.

A general (financial) power of attorney- This is often used for financial choices but can extend beyond that and touch on property management etc. If the principal is unable to comply with the provisions of the agreement, the agreement is deemed void and unenforceable.

Requirements needed to sign the document- The attendance of a notary public is required under Sections 21–2101 of the Code of Civil Procedure.

Restricted (Limited) Power of Attorney- A limited power of attorney may be granted for a single action or for a single authorization to carry out a specified task, such as signing a document. Once that activity has been completed, the limited power of attorney expires.

Declaration of living will- Signing a living will allows you to make choices about your end-of-life care and treatment alternatives, such as organ donation. Your preferences are put in writing and given to your medical care team for them to follow when possible.

Medical Power of Attorney – This agreement delegated responsibility to someone else to make medical decisions on your behalf in the event that you are unable to do so yourself. It comes into effect once you are considered incompetent or become disabled.

Minor (Child) Power of Attorney- It may be used to assign responsibility for medical and daily care for children under the age of majority. It has a limited effective period of up to one year. If you need it to last longer, you may need to set up a temporary guardianship.

Power of Attorney for real estate matters- Using a real estate power of attorney, it is possible to designate a third party to manage the sale or lease of a piece of real estate on your behalf.

Power of Attorney Revocation Form - A power of attorney designation may be withdrawn at any time by the named principal. This document allows you to identify the agent, principal, and specific power of attorney and revoke the authority it grants.

Power of Attorney for Motor Vehicle (Form DMV-VSPA-01) - Nominates someone else to take care of the registration of a vehicle, as well as the purchase and sale of autos, if necessary (Form DMV-VSPA-01).
