Durable Power of Attorney (statutory) – You will be able to appoint someone to handle your financial decisions by filling out this form. If this form is completed, it will go into effect the moment the person is rendered incapable, and it will last indefinitely.

General Power of Attorney (financial): Using this legal form, a person can delegate financial management to a friend or relative. These documents lose their validity if the "principal" is rendered incapable, unlike the durable POA.

This document is available in Adobe PDF and MS Word format, respectively. There are two different formats of this document: MS Word and Adobe PDF. Requirements for document signatures ( 18-16-2): Two witnesses a minimum, as well as a notary public, are needed.

Limited Power of Attorney: Using this form, a person who only needs momentary or temporary representation appoints a substitute to act on their behalf for the duration of the task.

Downloadable versions of the document include MS Word and Adobe PDF. 18-16-2 stipulates that two witnesses and a notary public must attest to the signature.

Medical Power of Attorney - If a patient is unable to make medical decisions for themselves, they can appoint someone to consult with their doctors on their behalf using this legal document.

Adobe PDF is the available download format. Requirements for signing ((§ 23-4.10-2): Two witnesses are required. Although notarization is an option, it is not required.  

Advance Directive - An advance directive is a legal document that allows a principal to appoint an agent and put their end-of-life treatment preferences in writing. The agent will help to uphold the decisions made and handle any issues outside of the purview of the written instructions. 

Minor Child Power of Attorney - Through this document, a person can authorize another to take temporary custody of their minor children while they are away and to make decisions about their education and health.

Both MS Word and Adobe PDF versions of the document are available for download. There are no prerequisites for signing, specifically. However, it is advised that three people sign the document: a notary public, two witnesses, and the caregiver. 

Real Estate Power of Attorney - This document is used to appoint an agent (not necessarily a real estate agent) to handle transactions related to landed property. This includes buying property and even selling property.

Power of Attorney Revocation Form – This form allows you to revoke an existing power of attorney. You’ll need to add the details of the original power of attorney so it’s clear which document should be canceled.

Downloadable versions of this document include MS Word and Adobe PDF. Signing is not subject to any particular conditions. Regardless, the principal's signature must be witnessed by a notary public.

Vehicle Power of Attorney - With the aid of this document, a person can name a representative to represent them in matters involving vehicle registration and titling before the Pennsylvania Department of Transportation.

It is available as an Adobe PDF or an MS Word document for download. Signatures require notary acknowledgment and two witnesses.
