New Jersey Living Will Form (Instruction Directive)

A New Jersey living will allows a person to refuse life-sustaining care in the case of an incurable and irreversible illness, disease, or condition. Conditional incurable and irreversible conditions are specified by the person who is putting the order in writing. Once prepared, a living will be signed by two (2) witnesses or a notary public before it may be considered valid.

Generally speaking, a living will or "instruction directive" is a written declaration of a declarant's health care choices if he or she becomes incapacitated.

How and what to put into writing

Step 1: You must, first of all, Establish the Declarant- A number of procedures must be followed to establish the identity of a Declarant in a judicial proceeding. Start by providing the name of the declarant.

Step 7 – Signature and Witness - A copy of this form should be handed to a doctor, a family member, or a friend, and it must be signed and witnessed. Please submit the following information to get an original copy of this document for the people or persons you want to have it sent to:

Step 8 – The Declarant’s signature – First and foremost, declarants must read and sign a declaration stating that they understand the conditions of their end-of-life options and that they have no objections to them. This is the last stage before they are allowed to sign the document. If the Declarant agrees with the statement after reading it, they must provide the following information to the government:

·         Date the signature in dd/mm/yyyy format

·         Declarant’s Signature should be provided

·         Detailed Address

·         City

·         Specify the State

Step 9-The signatures of the witnesses must be provided here: – The signatures of the witnesses must be witnessed by the Declarant.
