"Declaration" signifies a report, assertion or statement that is duly witnessed and is intentionally made by the declarant, approving the withdrawal or denial of life-supporting methodologies, as per the necessities of this Subpart. A declaration may recorded as a hard copy, orally, or by different methods for nonverbal communication.
Stage 1 – Establishment of Declarant/Principal – The data below should be duly entered:
The top of the document must be dated in this pattern: dd/mm/yyyy
The complete legitimate name of the declarant/principal must be entered.
Stage 2 – Declarant/Principal End of Life Selections – The next segment of the document must be carefully evaluated by the Declarant
Once evaluated, the end-of-life treatment or therapy must be selected by initialing the line that precedes the choice you want to make.
